Please fill up this form for your child’s iREAD (2022 Semester 1) registration. Semester 1 is from Jan to May 2021, with 2 sessions a week.

COST: $30/month, subsidies available.

  • I fully understand and agree that the personal information which I have provided or are collected during C3 activities may be collected, used or disclosed for the purposes of:
    (a) Engaging me regarding C3 programmes, services and activities;
    (b) Record keeping, storage, audit and promotion of C3 and her programmes;
    (c) Inclusion in publicity videos, brochures and other collaterals. Information collected may include photographs, audio recording, videos or other media.
    (d) Updating partners and sponsors regarding the progress of students.

    I trust that the information will be used strictly for the purposes stated. I agree for C3 to contact me for purposes relating to C3 programmes, services and activities and maintaining the ongoing relationship with C3 until/unless I state otherwise.

  • I am registering my child in iREAD (Semester 2). I will ensure that my child attends the classes as scheduled. I confirm that my child’s participation in the programme and the related activities is entirely voluntary and I accept all risks involved therein. Accordingly, I hereby indemnify and shall not hold liable Calvary Community Care (C3) and/or any of their employees, volunteers or partners for any loss, damage (including indirect or consequential loss and/or damage), injury or illness suffered by me or my child as a result, directly or indirectly, of participating in the programme.
